Any Activity Sponsored by Wilco Football
Athletes particpating in any activity sponsored by Wilco Football, must fill out and sign

liability waiver (download all encompassing waiver.pdf at the bottom of the page).
Only one is required no matter how many activities the athlete attends. A new waiver needs to be signed each calendar year.
Spring Football
The following must be turned in by March 10. (missing this deadline will result in a $50 additional fee and a possible delay in starting practice with pads). Forms are downloadable at the bottom of the page.
- Fees - $125 (Make checks payable to Scott Diesing. A $75 credit will be applied to the fee for fall football for all players who play in the spring and the fall.)
- Registration (Registration.pdf - please include email for Mom, Dad, and player. Also indicate if your phone is a cell phone and if it will accept text messages.)
- Sports Physical1 (phys_eval_medical.pdf - allows you to fill in electronically and print)
- Medical and Liability Release (Medical Consent Form and Liability Release Agreement.pdf)
- Copy of Medical insurance card
- Rules (Rules.pdf - return page 5 only)
- Get measured for helmet, pads, and game pants.
- Equipment agreement (Equipment return agreement.pdf)
Jr. High Skills Camp
Camps will be held in April and July. Forms are downloadable at the bottom of the page.

Fall Football2

Notes
- TCAL requires a physical every two years. We recommend one every year. If you have one on file with us that is within two years (including the full dates of participation) we do not need another one.
- Forms that were submitted for spring football are sufficient. The only new document required is the birth certificate.
